Rental Reservation Request

If you are ready to request your massage or spa equipment rental, please use this form to submit your dates, quantities, and comments.  We'll follow up with you shortly to confirm availability and schedule your rental.  

Your custom e-mail estimate will follow shortly.

After your rental request is received, we will complete a custom estimate, which is delivered by e-mail in most cases.  The estimate includes rental costs and shipping, and will confirm the details of your rental.  If you prefer an estimate by mail, please note that in the comments section below.  You can expect your e-mail rental estimate to arrive within 24 hours.  Estimates are prepared Monday through Friday during business hours, 9-5 central standard time.  We are sometimes available for Saturday estimates, so please feel free to use this form or call us if you are working with short notice.

Before your rental is confirmed, we will also need a completed Rental Agreement (Pops up in new window) and payment, which you can provide with our Charge Info Form or call us with your credit card info at 800-687-5199.
